About The Position

Maintains anactive caseloadof clients who need case management and supportive counseling onan ongoing basis. Implements ongoing monitoring of clients’ needs, and evaluation of services. Exercise andpromotesa functional referral system with all departments withinCHIsystem to ensure quality care. Coordinates with various external resources to provide services for patients as needed. Maintains records of services provided to clients and completes records for funding programs,assistsin gathering and completes records for funding programs,assistin gatheringand compilinginformation for internal and external reports. Participates in patient/community education (internal and external) and home visits when necessary. Communicates to supervisor/appropriatestaffpertinent information. Participates in Quality Assurance Program and continuing education asrequired. Maintains all licenses and/or certificates asrequiredforjob. Maintains 80% productivity monthly. Reports to work on time and ready to work with minimal absenteeism. Completes B&E (billing and encounter) forms within the same day of intervention. Providesaccurateandtimelydocumentation in patient charts within 24hrsof intervention. Adheres to Confidentiality Policies and Procedures / HIPPA Regulations. Performs other duties as assigned.

Requirements

  • High School Diploma or GED equivalent.
  • Knowledge of HIV transmission and prevention.
  • Maintain current CPR certification from the American Heart Association.
  • Verbal and written fluency in English.

  • Creole and Spanish desirable.
  • Knowledge of HIV/AIDS transmission/prevention desirable.
