About The Position

Community Mental Health Affiliates, Inc. (CMHA) is a private non-profit treatment provider headquartered in New Britain, with seven locations throughout the city and in Waterbury. We partner with clients and the community to promote recovery from mental illness and substance use, treating more than 7,500 adults and children each year. CMHA is looking to hire Full-time, 1st shift Peer Navigator for our Adult Outpatient Program. This position will be located at 233 Main St. New Britain, CT AND 255 Bank st. Waterbury, CT. This position will be Monday - Friday, 9:00am - 5:00pm. Compensation Range: The salary range for this position starts at $18.00 per hour. Compensation for each position is determined not only by years of relevant experience, education, and skills, but also by maintaining internal pay equity. Position Summary: The Peer Navigator provides initial and ongoing outreach and support to potential and current clients. They support, teach, advocate and empower individuals with behavioral health disorders; which includes developing person centered recovery goals. Liaison with local emergency rooms, detox centers and treatment facilities. They engage in traditional and recovery oriented systems navigation, coordination of services, skill building and eliminating any barriers to care. Goal is to empower and educate clients to rediscover their ability to pursue and achieve a higher quality of life that allows them to prosper in the community. Partners with the VA and Veterans to navigate systems of care and coordinate services.

Requirements

  • Recovery Coach preferred.
  • Valid CT Drivers license
  • Must have Military experience.
  • Successful related work experience, preferably a minimum of 1-2 years.

Responsibilities

  • Selectively shares personal recovery experience to provide hope and empowerment to instill client change in individual and group modalities including 12 step groups.
  • Facilitates client support groups.
  • Builds trusting, recovery oriented rapport with individuals with mental health or co-occurring disorders and links them to wrap around services.
  • Provides input and advocacy by actively participating in community meetings to ensure the provision of integrated care.
  • Coaches and assists clients with all in-home challenges such as personal hygiene, nutrition, housekeeping, personal budget and household maintenance and repair.
  • Collaborates with care team on client engagement and treatment recommendations.
  • Provides encouragement and recovery knowledge to family members/natural supports.
  • Carries a caseload based on program need.
  • Ensures timely documentation is maintained in accordance with CMHA policies and procedures.
  • This position with work with Veterans in the New Britain and Waterbury office.
  • Performs other related duties as assigned.
