CMHA is looking to hire a full-time Lead Case Manager for our Community Support program (CSP). This position will be located at 233 Main St. New Britain, CT 06051. Monday-Friday, 9:00am-5:00pm. Position Summary : The Case Manager a ssists in overseeing program functions/operations in accordance with policies, procedures, internal processes, and external regulatory agencies, including but not limited to DPH, Joint Commission, DMHAS, DCF and Grant/Payer Sources under the supervision of Program Coordinator. Provides coaching to staff, as well as ensuring appropriate delivery of services to adult clients. Responsible for ensuring treatment is person-centered and recovery oriented. Responsible for a caseload of clients. Serves as an intensive case manager to support, teach, advocate for individuals with mental health, primary care and/or co-occurring disorders.
