About The Position

Bridges Healthcare, Inc. has an immediate opening for a Peer Case Manager for our Contingency Management Program. This position is 19 hours per week, non-benefited. $25.00 per hour PURPOSE and POSITION OVERVIEW To support activities and programming for Contingency Management that will facilitate and encourage individuals to become empowered in their recovery. Requirements

  • Bachelor's degree preferred, with major in human services; Certified Peer Support Specialist program completion required
  • Ability to work flexible schedule.
  • Experience working with individuals with mental health and substance use disorders.
  • Good communication and documentation skills.
  • Computer literacy.
  • Ability to prioritize and organize schedules/demands.
  • CPR/First Aid certified or willing to become certified.
  • Valid driver's license and access to reliable transportation.

Responsibilities

  • Assist with the implementation, operations and monitoring of the CM program in adherence with established procedures.
  • Attend all required CM trainings, coaching sessions and Readiness Team meetings.
  • Provide back-up coverage for the CM Manager.
  • Facilitate recovery focused activities to motivate consumers toward improving their quality of life.
  • Provide advocacy and education on treatment barriers and stigma.
  • Network and collaborate with community resources and providers.
  • Assist in the development of consumer's treatment plan.
  • Maintain client records.
  • Perform any other department or agency related duties as directed by supervisor.
