About The Position

The Pediatric Psychosocial Specialist is a member of an interdisciplinary pediatric care team and is responsible for addressing the psychosocial needs of patients and their families. This role conducts assessments, provides supportive counseling, coordinates resources and referrals, and advocates for patient and family needs. The specialist collaborates with internal teams and community resources to ensure appropriate psychosocial support and continuity of care.

Requirements

  • Pediatric and family experience preferred
  • Experience in supportive counseling in group and individual settings, and/or direct service with clients and their families in health care setting, experience in provision of case management service.
  • Strong interpersonal and communication skills
  • Ability to work with a wide range of constituencies in a diverse community
  • Ability to work flexible hours when needed
  • Ability to work within framework of multi-disciplinary team
  • Current licensure as a registered clinical social worker in state of Florida or License eligible
  • Current state of Florida driver’s license with acceptable driving record and reliable transportation.
  • Level II AHCA (Agency for Health Care Administration) Background Screening required.
  • Master's Degree in Social Work from an accredited school of Social Work and three (3) years direct service experience, LCSW preferred.
  • Must have medical clearance for communicable diseases before direct patient contact.
  • Current immunizations including but not limited to TB, Hepatitis, and Flu.

Responsibilities

  • Conduct psychosocial assessments to identify patient and family needs, risk factors, and support requirements.
  • Provide supportive counseling, crisis intervention, and case management services to patients and families.
  • Coordinate psychosocial interventions and facilitate referrals to appropriate community resources and support networks.
  • Develop and maintain collaborative relationships with community professionals and service agencies to support patient care coordination.
  • Participate as an active member of the interdisciplinary care team by sharing relevant psychosocial information and recommendations.
  • Advocate for patient and family rights, including support for self-determination and access to services.
  • Assist with transitions or changes in level of care by coordinating resources and providing support to patients and families.
  • Identify when specialized mental health services are needed and facilitate appropriate referrals.
  • Complete psychosocial components of comprehensive assessments and other required evaluations within established timelines.
  • Maintain accurate and timely documentation of assessments, interventions, and patient progress.
  • Participate in professional development activities, including training sessions, workshops, and professional meetings.
  • Provide consultation and guidance to interdisciplinary team members regarding psychosocial issues, family dynamics, and available resources.
  • Adhere to applicable professional standards and ethical guidelines in the delivery of services.
  • Perform other related duties as assigned.
