Primary care coordinator of interdisciplinary patient care activities and responsible for the management of patients throughout their continuum of care at St. Jude Children's Research Hospital. Responsible and accountable for high quality patient care through implementation of the nursing process including assessment of patient needs (physical, psychosocial, educational), design of a collaborative plan of care, implementation of the plan with appropriate delegation of tasks, coordination of team effort, and evaluation of patient progress towards outcomes. Acts as a patient/family advocate, she/he seeks opportunities to communicate and coordinate patient care activities throughout the continuum in collaboration with other members of the Health Care Team. The Bone Marrow Transplant unit serves patients who are being worked up for or have recently received transplants or patients who require cellular therapy. This is a unique patient population requiring care involving attention to detail, recognizing rejection or sepsis in early stages, and detailed patient education. The BMT unit is accredited by the Foundation of the Accreditation of Cellular Therapy (FACT) and provides data for the Center for International Blood and Marrow Transplant Research.
