About The Position

The care management position is responsible for assisting patients and families in navigating their healthcare. Their primary focus is high-risk patient populations within primary care pediatrics. Each role will encompass patient care as well as being the lead coordinator for all services related to the assigned patients; working closely with multiple agencies, specialty services, and providers. This role will be responsible for the patient's individual needs as they relate to age, physical, psychosocial, cultural, and education.

Requirements

• R.N. NYS license or eligible
• Bachelor's degree in Nursing or healthcare related field
• minimum of three (3) years RN nursing experience required
• CPR-BLS is required by the time of appointment

Nice To Haves

• Master's degree in Nursing or healthcare-related field
• Pediatric and ambulatory experience preferred
