About The Position

Cowlitz Family Health Center is a Federally Qualified Health Center looking for a Parent-Child Assistance Advocate to provide compassionate support, resources, and guidance to families in need. If you’re ready to make an impact in your community, we want to hear from you! The Parent-Child Assistance Program (PCAP) is a paraprofessional intervention for high-risk women and their families. This position will be responsible for providing direct outreach, home visitation case management, and advocacy services to high-risk mothers who have used alcohol and/or drugs heavily during pregnancy, and to their children, partners, and extended families when needed. The PCAP Advocate maintains a professional, collegial positive relationship with staff and clients. Wage Range: $47,652 - $50,064 Per Year Schedule: Monday Through Friday 8:00 am - 5:00 pm FTE: 1.0

Requirements

  • 4-year BA or BS degree and 4 years of community-based experience working with high-risk populations
  • Experience on issues/projects working with prenatal substance abuse or equivalent combination of education and work experience
  • Strong communication skills
  • Demonstrated ability to effectively engage with high-risk mothers and their families
  • Ability to conduct interviews covering extremely sensitive and confidential issues
  • Strong organizational and time management skills
  • Minimum of 1-2 years of previous/related experience.
  • Strong customer service skills.
  • Valid, unrestricted driver’s license.
  • Must maintain all certifications necessary to fulfill the responsibilities of this position
  • Ability to work with a low-income, multi-ethnic patient population and a demonstrated ability to relate respectfully and effectively to people of diverse racial, educational and socio-economic backgrounds.
  • Must be a confident, self-directed individual who can work within a team as well as independently
  • Strong communication, organization, documentation and record keeping skills necessary.
