About The Position

Position Purpose The primary purpose of this position is to perform and oversee resident personal care or to assist residents in performing personal care, including dining services, in accordance with Facility policies and procedures. Schedule is every Sunday, Monday, Tuesday - Day Shift 6am - 6pm.

Requirements

  • Employee must be at least 18 years of age, as position operates potentially hazardous equipment.
  • The employee must be able to perform multiple tasks simultaneously and work under time and/or deadline pressures.
  • While performing the duties of this job, the employee is frequently required to sit; use hands to finger, handle, or feel; reach with hands and arms; stoop, kneel, bend, squat, crouch, or crawl; talk and hear.
  • The employee must frequently lift up to 10 pounds, but rarely lift up to 20 pounds, and rarely lift up to 40 pounds.
  • The employee must occasionally reach above shoulder height, frequently reach from chest to shoulder, and frequently reach below waist level.
  • The employee must occasionally carry up to 12 pounds but rarely carry up to 25 pounds.
  • The employee must frequently push / pull up to 20 pounds, but rarely push up to 40 pounds and rarely push / pull up to 40 pounds.
  • Specific vision abilities required by this position include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
  • Graduation from high school or GED preferred.

Nice To Haves

  • 2 years preferred in a memory care setting
  • 2 years experience in healthcare.

Responsibilities

  • Accurately and safely perform resident care, in accordance with residents’ service plan, resident. Assignment Sheets and worksheets, and nurses’ instructions.
  • Follow resident Service Plans and give input for changes in writing.
  • Administer PRN medications, per procedure.
  • Answer residents’ pendant alarms and emergency call buttons in a timely manner, then follow the incident reporting procedure.
  • Check residents’ medications drawer and initial on the Medication Sheet when they have taken their medicines; remind residents to take medications and/or give assistance when needed.
  • Straighten and maintain Assisted Living Memory Care area and equipment storage areas in neat and orderly condition.
  • Give incoming staff a report at the end of assigned shift.
  • Accurately record resident care on Resident Assignment Sheet.
  • File documentation in resident charts as needed.
  • Inform nurses, in writing, of improvement or decline in resident condition on a timely basis.
  • Charge out billable supplies.
  • Answer Assisted Living Memory Care telephone when nurse is unavailable.
  • Practice appropriate phone and pager etiquette.
  • Communicate with supervisor and participate in training and/or meetings that help facilitate departmental communication process.
  • Attend and participate in in-service educational classes, on-the-job training programs, etc., as scheduled or required.
  • Attend staff and departmental meetings, as directed.
  • Communicate with others in a considerate manner.
  • Function effectively as a team member.
  • Maintain understanding, patience, and tact in dealing with residents, resident families, and coworkers, and be cooperative.
  • Maintain good working relationships with other staff.
  • Respect residents’ personal items.
  • Ensure maintenance of a safe living and working environment for residents and staff.
  • Work independently and maintain good problem-solving skills.
  • Maintain regular work attendance and follow established work schedule.
  • At times, hours and days of the week must be flexible to meet scheduling requirements.
