Position Overview The Patient Support Specialist will serve formerly incarcerated individuals who also have chronic disease, mental illness, and or substance use disorder who meet the program requirements. Primary responsibilities will include connecting identified individuals to appropriate health care services and collaboratively create a comprehensive reentry plan. THIS POSITION OFFERS A 40-HOUR WORKWEEK, NO WEEKENDS, FULL BENEFITS PACKAGE, 12 PAID HOLIDAYS, BI-WEEKLY PAY. THIS POSITION WILL ALSO WORK AT OUR OTHER CLINICS (AND ACCORDING TO THE HOURS OF OPERATIONS) - WILSON COMMUNITY HEALTH CENTER AND HARVEST FAMILY HEALTH CENTER.

Requirements

  • High School Diploma or GED.
  • Personal history of incarceration.
  • Ability to operate office equipment such as computers and telephones.
  • Ability to navigate records keeping system and document work completed.
  • Valid NC Driver License.

Nice To Haves

  • History of work in a medical setting preferred, but not required.

Responsibilities

  • Provide support, empowerment, nonclinical education, and care coordination services to individuals released from incarceration within the last two years who have chronic disease(s), substance use disorder, and/or mental illness.
  • Review referrals for The University of North Carolina at Chapel Hill (UNC) Formerly Incarcerated Transition (FIT) program and determine the eligibility of participants based on program requirements.
  • Assist individuals with linkage to, and enrollment into, primary care services, substance use disorder treatment, and mental health services.
  • Conduct periodic assessments of an individual's substance use, physical activity, dietary habits, tobacco use, and social and economic resources.
  • Provide or arrange transportation for ambulatory patients between their homes and clinics, hospitals, or other social agencies when appropriate and necessary.
