Nascentia Health is seeking a Patient Services Coordinator to serve as the central liaison for coordinating home health services for our Certified Home Health Agency (CHHA) patients. This role ensures that patients receive timely, high-quality care by managing scheduling, communication, and service coordination.

Requirements

  • High school diploma or equivalent required (Associates degree in healthcare administration or related field preferred).
  • Experience in home health, scheduling, or care coordination preferred.
  • Strong organizational and problem-solving skills.
  • Excellent communication and interpersonal skills to interact with staff, patients, and families.
  • Proficiency in computer applications and electronic health records (EHRs).
  • Ability to sit or stand at a desk 90% of the day.
  • Frequent sitting, standing, walking.
  • Visual/hearing ability sufficient to comprehend written or verbal communication.
  • Ability to express oneself verbally and in writing.
  • Ability to use the telephone and communicate over the phone.
  • Extended periods of computer use.
  • Ability to lift and move objects up to 40 pounds.
  • Bloodborne Pathogens Exposure Category III - Employee who rarely has contact with blood and body fluids

Nice To Haves

Responsibilities

  • Coordination of Skilled Services Facilitate and oversee the assignment of patients based on care needs, availability, and regulatory requirements.
  • Communicate with CHHA teams to ensure seamless service delivery.
  • Monitor service schedules and adjust assignments as needed to address patient needs, staff availability, and agency capacity.
  • Maintain accurate records of aide assignments, service authorizations, and patient care plans.
  • Communication & Collaboration Serve as a contact between field staff, and supervisor team.
  • Collaborate with staff to confirm assignments align with plan of cares.
  • Communicate effectively with patients and families regarding scheduled services and any necessary adjustments.
  • Address and resolve scheduling conflicts or service gaps proactively.
  • Compliance & Quality Assurance Ensure adherence to all CHHA regulations, policies, and patient care standards.
  • Participate in quality improvement initiatives related to CHHA services.
  • Maintain confidentiality and compliance with HIPAA regulations.
  • Administrative Support Assist with documentation, data entry, and reporting related to CHHA services.
  • Verification of visits in EMR with contract agencies.
  • Support leadership in tracking performance metrics and service utilization.
  • Attend meetings and training sessions as required.
