General Purpose of Job: Responsible for providing constant direct observation of adolescent, adult, and geriatric patients who may be at risk for adverse events such as falls, self injury, or harm to others. Essential Duties: This job description is intended to cover the minimum essential duties assigned on a regular basis. Associates may be asked to perform additional duties as assigned by their leader. Leadership has the right to alter or modify the duties of the position. Responsible for specific aspects of direct patient care and monitoring focused on safety, under the direct supervision of a Registered Nurse. As directed by RN ambulates patient, assists with meals if there are no dysphagia concerns, uses appropriate distraction tactics when applicable ( i.e. conversation, walking, games). May be required to provide frequent physical redirection of patient. Documents observations accurately and timely on correct forms. Immediately communicates significant information to RN such as changes in the patient’s behavior or condition.
