The Patient Safety Manager RN has responsibility for and accountability for directing, managing, and overseeing the patient safety program at UVACH facilities as assigned. This role provides leadership and strategic direction to foster a culture of safety, proactively identifying risks and implementing initiatives to enhance patient safety and reduce harm. The Patient Safety Manager RN partners with clinical and operational leaders to develop, implement, and monitor a comprehensive patient safety program, including both strategic and operational plans. This role leads proactive and reactive safety efforts, including event analysis, risk assessments, and system-based improvements to reduce unintended adverse patient outcomes. The Patient Safety Manager RN designs, implements, and delivers ongoing education and training to staff and leadership to promote understanding of patient safety principles, human factors, and high reliability practices. This role collaborates across UVACH to drive process improvement, strengthen safety culture, and ensure alignment with regulatory and accreditation requirements.

Requirements

  • Bachelor of Science in Nursing (BSN) required
  • Minimum of 5 years of clinical healthcare experience required
  • Current Registered Nurse (RN) license required (Virginia or eligible for licensure in Virginia)
  • Strong knowledge of patient safety science, human factors, and risk management principles
  • Demonstrated experience conducting event investigations and risk assessments (CSA, FMEA, ACA, CCA)
  • Ability to perform detailed clinical chart reviews and translate findings into actionable improvements
  • Experience developing and delivering education and training to clinical staff
  • Strong leadership, facilitation, and project management skills
  • Excellent analytical, problem-solving, and critical thinking abilities
  • Effective written and verbal communication skills, including presentation to senior leadership
  • Ability to influence change and drive interdisciplinary collaboration
  • Knowledge of regulatory and accreditation standards related to patient safety
  • Strong organizational and time management skills

Nice To Haves

  • Master’s degree (MSN, MHA, MPH, or related field) preferred
  • Experience in patient safety, quality improvement, or risk management strongly preferred
  • Experience leading event investigations (e.g., CSA/RCA) and proactive risk assessments (e.g., FMEA) preferred
  • Certification in Patient Safety (e.g., CPPS) preferred

Responsibilities

  • Lead and manage the patient safety program at assigned facilities
  • Conduct Comprehensive Systematic Analyses (CSAs) and ensure timely completion of action plans following serious safety events
  • Facilitate and lead Failure Mode and Effects Analyses (FMEAs) and other proactive risk assessments as directed
  • Perform clinical chart reviews to identify trends, gaps in care, and opportunities for harm reduction
  • Analyze safety event data to identify trends, root causes, and system vulnerabilities
  • Partner with leaders to implement sustainable corrective actions and monitor effectiveness
  • Develop and deliver patient safety education and training for clinical and non-clinical staff
  • Support development and sustainment of a Just Culture and high reliability organization
  • Collaborate with risk management, quality, infection prevention, and clinical teams to align safety initiatives
  • Ensure compliance with regulatory and accreditation standards (e.g., Joint Commission, Centers for Medicare & Medicaid Services)
  • Prepare reports and present patient safety data, trends, and improvement initiatives to leadership and committees
  • Lead or support safety committees and multidisciplinary workgroups
  • Drive continuous improvement through evidence-based safety practices and change management strategies
  • Demonstrates effective interpersonal communication skills

  • Effectively communicates safety data, risks, and improvement strategies to stakeholders
