About The Position

The Patient Safety Leader will be responsible for administering the Patient Safety Program on a day-to-day basis for Loyola Medicine (IL) and St. Joseph Health System (IN) . This role will partner with clinical and operational leaders across the organization to oversee the rollout of patient safety tools and high reliability practices. This role will interact with Trinity Health colleagues to ensure compliance with system requirements and provide updates to Regional Leadership regarding the barriers and success of the Patient Safety Program.

Requirements

  • Bachelor's degree required
  • Minimum Experience: 1-2 years of previous job-related experience required; 3-5 years preferred.
  • Past work in Risk Management, Process Improvement, Quality, or other demonstrated training and experience in Patient Safety
  • Current CPPS, CPHQ, or CPHRM preferred.
  • Will be required to earn CPPS within 18 months of hire if not already in place.

Nice To Haves

  • Master's degree preferred.

Responsibilities

  • Program planning, coordination and evaluation. Develops, plans, coordinates, implements, and manages the Patient Safety Program for Loyola Medicine and St. Joseph Health System
  • Coordinates, facilitates and/or educates colleagues and medical staff on their roles and responsibilities in the Patient Safety Program, to include reporting of Patient Safety events, participating in Patient Safety activities, utilization of Patient Safety tools, and the characteristics of high reliability organizations.
  • Ensures effective feedback on lessons learned and process/system improvements.
  • Collaborate with Risk Management and Quality for reviewing, investigation, analysis, action planning, and tracking of improvements associated with patient safety events and near misses.
  • Participate on quality committees.
  • Provide oversight of annual Culture of Safety Survey. Analyze patient safety data to set meaningful goals for the Patient Safety Program.
  • Communicate with hospital and regional leadership regarding the status, barriers, and successes of the Patient Safety Program.
