Requirements

• RN License required.
• Home Health experience of knowledge of Clinical Terminology preferred.
• Responsible, always on time, resourceful, very accurate and detail-oriented.
• Flexible as to work schedule. Must be available to work on weekends, holidays and in emergency situations.
• Must be able to work under pressure and multi-task.
• Self-motivated with a strong work ethic and the ability to work independently AND as a team member.
• Demonstrate excellent verbal communications, interpersonal skills and empathetic listening.

Responsibilities

• Review all admission documentation in order to ensure the quality of the services to be provided.
• Responsible for coordination with all field staff that they follow agency’s policies and procedures.
• Responsible for reviewing the patient’s plan of care and ensuring that it gets properly implemented, including discussing it with the patients as necessary.
• Maintains daily communication with field staff in order to ensure that all services are provided as per patient’s expectations.
• Responsible for reviewing all physicians’ orders obtained by field staff.
• Verifies the proper delivery of daily services as scheduled, including unscheduled emergent visits in assigned community.
