About The Position

Community Healthcare Network is seeking a Full-Time Patient Referral Coordinator who will be responsible for coordinating referrals, scheduling initial appointments and follow-ups for patients.

Requirements

• High School Diploma or equivalent.
• Minimum of three (3) years office experience.

Nice To Haves

• Bilingual in Spanish (preferred).
• Knowledge of Microsoft Word and/or WordPerfect.

Responsibilities

• Secures referrals and clinic appointments for patients.
• Follow-up with referrals to ascertain attendance and obtain a completed report.
• Develops working relationship with referral services to assure that rapid referrals appointment are made.
• Works with Providers, Nurses and Center Directors.
• Maintains Patient Referral Database.
• Typing of correspondence, faxing.
• Answering of telephones.
• Facilitates related services for health center patients as appropriate with respect to their confidentiality and privacy.
• Performs other related duties, as assigned.
