Job Summary Coordinates all processes, support, communication and documentation involved in placing patients in facility in appropriate care setting. Patient Placement Coordinators also required to recognize and uphold all regulatory requirements, while simultaneously updating key stakeholders of timelines. Follows patient placement criteria to place patients in appropriate care setting. Accurately enters all required data into EPIC documentation system.

Requirements

  • HS Diploma or GED
  • One to two years of direct clinical experience, or one to two years of full time direct administrative healthcare experience (such as Nursing Unit Secretary or Patient Access Representative).
  • Working knowledge of medical terminology, excellent computer skills, customer service, ability to multi- task efficiently and effectively.

Responsibilities
