About The Position

POSITION SUMMARY: The Patient Navigator assists with diagnosis of patients in need of testing for hepatitis C, B, and HIV. As a grant funded position, the patient navigator (PN) is to work in the emergency department (ED) at Hudson Regional Health (HRH), using United States Preventative Services Task Force (USPSTF) guidelines, to identify and facilitate testing of patients meeting testing criteria for hepatitis C, hepatitis B and HIV. The PN will work with ED staff to verify if patients meeting USPSTF criteria (age>18) are asked if they had been tested for hepatitis C, hepatitis B and HIV. For untested patients meeting criteria, the PN will work with ED staff to order testing. The PN will also verify country of birth of patients seen in the Hoboken ED. Patients meeting demographic testing criteria for hepatis B infection risk will have a serum hepatitis B surface antigen ordered. The patient navigator will assist ED staff to use appropriate ICD- 10 coding for testing. The PN will work with the project director to review monthly reports of patients with a FIB4 score >3.0 identified within our hospitals to identify those patients with advanced liver damage and screen them for hepatitis C, B, and HIV. The PN will collaborate with outpatient substance abuse services and inpatient substance abuse services within our systems to increase hepatitis C, hepatitis B ang HIV testing. The PN is responsible for disclosing positive results to patients newly diagnosed or reengaged into care and will provide linkage to care (referral to the appropriate infectious disease or liver clinic). The PN is responsible for compiling and maintaining data to transmit to the grant sponsor. The PN is responsible for the correction of data errors, and the validation of submitted data.

Requirements

  • Monday though Friday - 8:00 AM to 4:30 PM
  • Associate degree in healthcare or public health related field, or Medical Assistant plus (2) years of experience with healthcare clinic operations in a patient-facing role.
  • Knowledge of HIV fundamentals and Hepatitis disease.
  • Demonstrated interest in and knowledge of community health care and primary care.
  • Electronic health record (EHR) experienced required.
  • Proficiency with Microsoft Word, Excel and databases required.
  • Ability to collaborate with physicians, staff, and external organization.
  • String communication skills with diverse populations required. Solid relationship- building and people skills. Excellent time management skills. Excellent coordination skills including multitasking and setting priorities on work assignments.
  • Knowledge of medical terminology and organization services.
  • Outstanding verbal and written communication skills, customer service skills, phone etiquette and works well in fast-paced dynamic atmosphere.
  • Ability to multi-task and work independently while being detail orientated.
  • High degree of independence, flexibility, initiative, and commitment.
  • Demonstrated awareness and value of cultural competence.
