About The Position

Provides care coordination duties as well as conducting navigation and eligibility assistance for patients at UH Health Family Care Center. Performs outreach, enrollment activities, and health education initiatives. Helps move program efforts forward, across programs or affiliated projects.

Requirements

  • Requires a basic knowledge of grammar, spelling, punctuation and simple mathematical functions like percentages, ratios, etc. as might normally be acquired through attainment of a high school diploma or GED.
  • Requires a minimum of one (1) year of directly job-related experience.
  • Requires technical certification or licensing directly related to the job as specified on the job posting details.
  • Community Health Worker Certification is a requirement.

Responsibilities

  • Ensures patients complete screenings, assessments or surveys and data is collected, and entered in a timely manner and utilizing safety protocols.
  • Guides patients to resources that address Non-Medical Drivers of Health (NMDOH) needs and ensures that cases are closed
  • Identify agencies that can provide resources to help fill in gaps for care and Non-Medical Drivers of Health (NMDOH).
  • Assist patients with managing chronic diseases to receive health services by breaking down barriers to care.
  • Community outreach on behalf of the health center, providing education about the clinic, chronic disease management, physical and mental health well-being
  • Maintains partnerships with community-based organizations, local schools/ school districts or health and community non-profits or stakeholders in the UH Health Family Care Center community/neighborhood service area
  • Maintains case record documents and information as it relates to UH Health Family Care Center initiatives and programs.
  • Provides information as needed to clinic staff members regarding patient's health needs or special cases in the community.
  • Identifies patient compliance barriers, still ensuring the quality-of-care coordination delivery.
  • Prepares reports of tasks and activities and keeps complete records of outreach events, activities, and contacts along with people reached.
  • Provides updated inventory of health education materials and community resources in the area.
  • Prepares, organizes, and executes care coordination initiatives for team members.
  • Assists with the implementation and ongoing monitoring of care coordination delivery.
  • Performs other job-related duties as required.
