About The Position

ReKlame Health is seeking a Patient Navigator Intern to support their Health-Related Social Needs (HRSN) care navigation program. This role involves working with clinical and operations teams to help patients overcome real-world barriers such as housing instability, food insecurity, and transportation gaps, which can hinder consistent, quality care. The position functions similarly to a case manager and is ideal for individuals pursuing careers in community mental health, social work, public health, or healthcare operations. The internship is fully remote with one required in-person training in New York City, full-time (40 hours/week) and compensated at $20/hour.

Requirements

• A current undergraduate or graduate student, or recent graduate, in social work, psychology, public health, healthcare administration, community health, or a related field.
• Genuine interest in community mental health, care navigation, or healthcare support
• Comfort discussing sensitive topics with empathy and professionalism
• Strong communication, interpersonal, and organizational skills
• Ability to work independently in a remote environment and stay accountable
• Openness to feedback, supervision, and mentorship
• Willingness to learn new systems and remote workflows quickly

Responsibilities

• Screen patients for health-related social needs: Conduct structured assessments using established screening tools and internal workflows.
• Connect patients to community resources: Help individuals navigate housing programs, food access, transportation support, and other essential services.
• Provide ongoing navigation support: Follow up with patients to ensure continuity and address emerging barriers as they arise.
• Collaborate across teams: Partner with clinical and operations staff to support individual care plans and document progress accurately.
• Support community outreach: Participate in engagement efforts to reach patients who may not yet be connected to services.
