The Patient Flow Facilitator-Licensed Vocational Nurse (PFF-LVN) is IV Therapy and Blood Withdrawal Certified in California and carries out patient care responsibilities under the supervision of a Registered Nurse (RN) and within the LVN Scope of Practice and in accordance with Loma Linda University Medical Center (LLUMC) and department established policies, procedures, protocols, techniques and guidelines. The PFF–LVN does not have a physical home unit, but instead, functions out of a home department and readily floats to all units of the hospital for full or partial shifts, or the hospitality/discharge lounge, the primary focus is patient flow. Assists with patient admissions, transfers and discharges by transporting patients to assigned units or transporting discharge patient to designated area, assists the RN with gathering data for the development of the care plan on admissions. Provides discharge teaching and reviews the after-visit summary with patient, family or caregiver as requested. Seeks information and clarification of duties and responsibilities when necessary. Exhibits interpersonal skills that portray professionalism and maximize excellence in customer service, safe patient care and professionalism. Contributes to a work environment of caring and cooperation among a culturally diverse workforce and patient population. Performs other duties as needed.
