About The Position

The Patient Engagement Coordinator/ Medical Adherence Coordinator is responsible for engaging patients in care through a data-driven approach to prevention, chronic disease management and medication management through the medication adherence program. The Coordinator will work with primarily low-risk patients, but may interact with moderate risk patients, and will be supported by the clinical pharmacists, nursing and social work resources to ensure that patients are able to access the care they need. The Coordinator develops effective interpersonal relationships with patients and works collaboratively with the interdisciplinary care team as well as pharmacies and community-based resources to improve health outcomes. The Patient Engagement Coordinator reports to the Senior Director of Condition Management and Population Health and Clinical Pharmacy Manager of Population Health.

Requirements

  • Bachelors degree, preferably in a health-related profession or combination of applicable experience and education.
  • 1-3 years relevant experience in healthcare, patient navigation or related field preferred.
  • Fluency in English
  • Intermediate skills using Microsoft Office.
  • Familiarity with electronic medical record and scheduling systems.
  • Effective communicator excellent written and verbal communication for use with patients, staff, and vendors.
  • Organized detail-oriented; consistently meets and tracks timelines and objectives.

Nice To Haves

  • Fluency in Spanish a plus.

Responsibilities

  • Conduct patient and pharmacy calls to close medication adherence care gaps.
  • Engage patients in conversations to assess medication adherence, identify barriers, and provide education on the importance of consistent medication use.
  • Assist patients with navigating pharmacy services, including home delivery services.
  • Collaborate closely with pharmacists, pharmacy interns, and other clinical team members to escalate cases requiring additional clinical intervention or support.
  • Conduct outreach calls to patients, caregivers, and pharmacies to confirm medication access, resolve prescription issues, and coordinate refills.
  • Document all patient interactions, adherence assessments, and follow-up actions accurately in the electronic medical record (EMR).
  • Track and report patient adherence trends, providing feedback to the care team to inform program improvements.
  • Support other patient engagement initiatives within the ambulatory pharmacy or population health teams as assigned.
  • Works to improve patients’ experience before, during and following engagement with the health system to provide care continuity.
  • Interacts with patients and caregivers as appropriate to ensure continuity of care, support adherence to care plans, and identify barriers that prevent adherence to care plans.
  • Follows the patient’s progress, identifies potential concerns, and collaborates with care team members to overcome social, financial or medical barriers.
  • Assists in medical record chart reviews for the purpose of quality audits, and assists in the collection of patient information for external providers, labs, and hospitals.
  • Educates patients regarding how to navigate throughout the health system.
  • Where deemed needed, may spend time helping practices with clinical workflows and dissemination of best practices.
  • Performs other duties as required.
