About The Position

Suncoast Hospice, a part of Empath Health is seeking a caring and compassionate Patient Concierge. A first responder from our agency who is passionate about the services we offer, and works directly with patient/family/authorized representative, or any other customer to insure optimal access. Schedule is 40 hours per week; Friday - Monday 9:00am - 8:00pm Since 1977, Suncoast Hospice has been honored to serve our neighbors across Pinellas and Hillsborough counties with compassionate end-of-life care. Requirements

  • Education and/or Experience: High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience.
  • Excellent verbal and written communication skills.
  • Knowledge of medical terminology.
  • Computer skills.
  • Knowledge of Empath Health/Suncoast Hospice programs.
  • Basic knowledge of reimbursement for services.
  • Working knowledge of community resources.
  • Ability to listen, assess needs, and coordinate care.
  • Must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion. Must be able to provide proof of automobile insurance, a copy of which will be placed in their employee file.

Responsibilities

  • Ability to work alongside the interdisciplinary team in all settings, while traveling to where the patient is.
  • Possesses the skills to educate about and conduct personal interviews with patients/families/authorized representatives while successfully articulating services.
  • Completes required documentation in real time by being accurate, thorough and detail-oriented from referral through admission
  • Able to assist with inquiries/education regarding self-determined life closure issues (Advance Directives), while also being knowledgeable about FL Proxy Guidelines to ensure proper signage of all forms related to admission process.
  • Maintains and continually builds rapport and trust with our community partners, community agencies, physicians, hospitals, and nursing homes.
