About The Position

Harvest Healthcare Solutions has immediate opening for Patient Care Navigators to support clients enrolled in the CalAIM Enhanced Care Management (ECM) and Community Supports Program. Patient Care Navigators play a critical role in helping clients—primarily seniors—stay connected to medical care and essential community resources. This position works collaboratively with Care Coordinators, Community Health Workers, Program Directors, and healthcare providers to address barriers to care and improve health outcomes. This is a part-time, community-based role that includes both telephonic support and in-field outreach throughout Plumas County.

Requirements

Candidates must possess a valid California driver’s license and meet the State’s automobile insurability requirement.
Candidates must be able to pass a DOJ criminal background check.
Community Outreach: 1 year (Required)

Associates or bachelor’s in social or human services preferred
Willingness to travel: 25% (Preferred)
Travel within the designated county areas

Responsibilities

Provide care navigation services to CalAIM ECM and Community Support members
Build trusting, supportive relationships with clients to promote engagement in medical and social services
Maintain timely and accurate documentation in compliance with program and state requirements
Serve as a culturally responsive advocate for clients and their healthcare needs
Communicates with Care Team members on a routine basis to support care delivery for patients.
Identify and connect patients to resources for all clients to overcome barriers to care, such as transportation, housing, food, and other social service resources.
Schedule and attend primary care physician appointments to review and update care plans with the Care Team.
Ability to assess for and make appropriate referrals for any identified mental health or psychosocial problems.
Ability to maintain client case records in a clear and concise manner in database.
