Requirements

  • High school diploma or equivalent required.
  • Bachelor's degree preferred.
  • MA, LPN, BSW, or Clinical Dietitian and appropriate accreditation/licensure preferred.
  • Experience working with patients who have chronic and behavioral health needs.
  • Demonstrated experience working as part of a multidisciplinary clinical team.

Responsibilities

  • Cares for patients as part of a care management program.
  • Responsible for patient caseloads through in-person, telephonic, and electronic communications.
  • Conducts outreach calls with patients and caregivers to encourage program participation.
  • Assists patients with self-care needs including addressing language and cultural barriers as well as coaching patients to meet personal and clinical goals.
  • Identifies patients in need of intensive case management.
  • Supports patients in scheduling provider visits, arranging transportation, and creating office visit agendas (what to ask, what to expect, etc).
  • Educates patients about preventative monitoring and caregivers about system response plans.
  • Helps patient access community and governmental based service agencies including possibly filling out patient paperwork.
  • Documents activities and reconciles patient information to support productivity of clinical care team.
  • Collaborates with the multidisciplinary clinical care team.
  • Administers program evaluation and conducts patient interviews.
  • Evaluates program processes and provides recommendations for improvement.
