About The Position

Patient Care Navigator - Gastrointestinal Lab (GI) Department: 37700 Advocate Lutheran General Hospital - Administration: Surgery Status: Full time Benefits Eligible: Yes Hours Per Week: 40 Schedule Details/Additional Information: Lead the Direct Access Screening Colonoscopy (DASC) program for Lutheran General Hospital Pay Range $41.10 - $61.65 Major Responsibilities: Navigation and Support to Patients 5)Provides education and support to patients and families 1)Guides patients through the healthcare experience utilizing a relationship based approach. 2)Navigates patients through the care process to ensure a positive patient experience and smooth transition through the continuum of care. 3)Serves as a liaison between the patient, physician office, care management and hospital services for all service lines to enhance continuity of care and create long term relationships with patients 4)Connects patients to services and follows up with patients to ensure satisfaction with the services and provide additional follow up as needed Coordination of Patient Navigation Program 1)Maintains an ongoing knowledge of Institute and Enterprise Service Line strategies and offerings and uses this information to guide patients through and to our services 2)Works with service line leaders to integrate current and newly developed programs into the navigation program 3)Participates in ALGH Strategic Planning Meetings and provides input from the patient and provider perspective 4)Develops a process for patient referrals including all stakeholders such as physicians, community groups, and ALGH associates and maintains data regarding referral sources 5)Creates and maintains statistical reports to analyze program productivity and success including financial, volume, and satisfaction data Community Outreach 3)Participates in project teams and committees to improve linkage for the navigation program 1)Participates in community events to create awareness of patient navigation services 2)Functions as a resource to physicians, associates and departments regarding care requirements of patients and programs Quality 1)Create an environment that is focused on service excellence, customer satisfaction, and competence. 2)Provide feedback to providers, service line leaders and hospital associates regarding the patient experience 3)In conjunction with service line leaders and hospital staff participates in the planning and organization of special events, community programs, open houses and marketing strategies. 4)Prepares reports and statistics for administrative purposes 5)Develop quality indicators to assess performance and patient outcomes related to the navigation experience Licensure: Nurse, Registered (RN) Education/Experience Required: BSN with 5 years experience Knowledge, Skills & Abilities Required: Strong communication skills coupled with strong customer service focus RN Physical Requirements and Working Conditions: Experience working with patients and communicating in an empathetic manner with patients and their family members with an emphasis on strong listening skills Able to convey information to individuals clearly taking into consideration the learning needs of the individual to ensure understanding. Able to adapt to the changing healthcare environment. Dedication to providing superior customer service If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Requirements

  • Nurse, Registered (RN)
  • BSN with 5 years experience
  • Strong communication skills coupled with strong customer service focus
