Patient Care Manager/In-Patient Unit (IPU) Administrator Job Description DEPARTMENT: Triage/Admission Department & Alexander Cohen Hospice House (ACHH) REPORTS TO: Chief Clinical Officer (CCO) SUPERVISES: Triage staff & IPU Interdisciplinary Team (Nursing, Social Work, Chaplaincy), Janitorial, Clerical Support, and Administrative Personnel. SUMMARY Patient Care Manager/IPU Administrator is a California-licensed Registered Nurse responsible for the 24-hour clinical, operational, and business management of the Triage Department and Alexander Cohen Hospice House for Community Hospice & Health Services. This role ensures the delivery of safe, high-quality hospice services in the outpatient environment as well as in the free-standing inpatient environment, maintaining strict compliance with Medicare Conditions of Participation ($42 ext CFR Part }418$), California Code of Regulations ($Title ext 22}$), and GIP $418.100$ standards. 1. Clinical Leadership & IDT Oversight Team Leadership: Facilitates the Interdisciplinary Team (IDT) meetings, ensuring comprehensive Plan of Care (POC) development and seamless communication between nursing, social work, chaplaincy, and facility partners (SNFs/RCFEs). Direct Support: Provides daily clinical consultation and maintains the competency to perform direct RN duties (including GIP/Respite coordination and meal planning) during emergencies or staffing shortages. Contracted Services: Manages relationships and quality assurance for contracted vendors, including DME, pharmacy, and infusion services. Fiscal Responsibility: Responsible for the Triage/Admission team & ACHH financial budgets, maintaining cost controls, and fiscal oversight. 2. Personnel Management & Staff Development Staffing & Scheduling: Manages 24-hour staffing, on-call rotations, and weekend coverage to meet fluctuating census needs. Responsible for time and attendance review, monitoring and approval. Performance Lifecycle: Leads recruitment, orientation, and retention efforts. Responsible for educating, evaluating, andâin collaboration with the CCO, Clinical Educator and HRâdisciplining or terminating personnel. Growth: Conducts frequent supervisory visits and competency validations, ensures staff maintain attendance at mandatory in-services. 3. Regulatory Compliance & Survey Readiness Governance: Reviews and revises clinical policies with the clinical management team, to ensure they reflect actual practice and comply with Medicare Conditions of Participation (CoPs), Medi-Cal, and California licensing laws. Audit Management: Performs routine chart audits and focused reviews to identify performance gaps. Prepares the team for unannounced audits by ensuring "survey-ready" documentation and clinical standards. Safety: Enforces 100% compliance with HIPAA, infection control, and universal precautions to maintain a safe environment for staff and families. Other Duties-as assigned by the Chief Clinical Officer. 4. Quality Improvement & Administration Performance Improvement (PI): Actively participates in the QAPI & Safety Committees, providing data and statistics to identify and resolve quality & safety issues. Continuity of Care: Oversees the admission/discharge process. Facilitates communication between the ACHH IDT, outpatient teams, and community services (SNFs/RCFEs). On-Call & Liaison: Provides administrative on-call coverage on a rotational basis and acts as the primary liaison for facility leadership to resolve care coordination or communication issues.
