About The Position

The Patient Care Coordinator acts as a key liaison between patients, healthcare providers, and external facilities. This role is responsible for coordinating care activities such as referrals, scheduling, medications, and authorizations while ensuring patients receive timely, accurate, and empathetic support throughout their treatment process.

Requirements

  • High school diploma or GED required; Bachelor’s degree preferred
  • 2+ years of experience in customer service or a healthcare setting preferred
  • 1+ year of experience using EHR systems (athenaOne preferred)
  • Solid understanding of medical terminology and insurance processes
  • Experience working remotely in a high‑volume phone environment preferred
  • Strong communication, organizational, and problem‑solving skills
  • Proficiency with Google Workspace and Microsoft Office
  • Ability to multitask, manage time effectively, and maintain attention to detail
  • Patient‑focused mindset with a commitment to confidentiality and HIPAA compliance
  • Must reside and work in PST or MST time zones

Responsibilities

  • Serve as the primary point of contact for patient questions related to referrals, lab work, imaging, medications, and treatment plans
  • Coordinate care with healthcare providers, facilities, pharmacies, and insurance partners to ensure timely delivery of services
  • Schedule patient appointments and manage requests using appropriate systems and resources
  • Communicate status updates to patients and partner facilities regarding labs, imaging, and procedures
  • Maintain accurate and up‑to‑date patient records, including demographics and documented interactions
  • Utilize critical thinking to resolve scheduling, authorization, or care coordination issues
  • Monitor patient satisfaction and escalate concerns when appropriate
  • Deliver professional, empathetic service in all written and verbal communications
  • Perform additional duties as assigned
