About this Role: Appointment Coordination & Patient Tracking – Schedule and monitor medical and behavioral health appointments, track patient progress, and support provider tasks and orders. Care Coordination & Case Management Support – Assist the Prevention Case Manager with patient care coordination, follow-up, and addressing barriers to treatment. Documentation & Communication – Process and manage patient records, referrals, and provider orders, including scanning, faxing, and routing documents. Regulatory Compliance & Customer Service – Ensure adherence to policies, maintain high-quality patient interactions, and facilitate connections to resources that address Social Determinants of Health. Team Collaboration & Additional Duties – Work collaboratively with colleagues, provide mentorship, and support organizational goals as needed. Requirements

  • Minimum of High School Diploma or GED
  • 2 years professional experience in a health or behavioral health practice preferred
  • This position requires a Level 2 background screening through the Florida Care Provider Background Screening Clearinghouse. For more information about the background screening process, visit https://info.flclearinghouse.com [https://info.flclearinghouse.com/].

Responsibilities
