Check us out online at www.capstonehospice.com Capstone Hospice is seeking a dedicated, and empathetic Patient Care Coordinator to join the administrative team. This position requires someone that is highly organized, detail-oriented and has experience using Microsoft Excel. The ideal candidate excels in multitasking, communication, and workflow management in a fast-paced environment. The PCC is responsible for facilitating the administrative tasks of the dedicated clinical team by supporting the Capstone clinical team members who are in the field caring for patients during life's most sensitive transition. This role also supports calls coming in from families and patients with questions and concerns. The Patient Care Coordinator will work independently with assigned tasks while working in a collaborative team environment. This position will report on-site in the Capstone office located in Peachtree Corners, working Monday - Friday, 8:00am-5:00pm. Requirements

  • High school graduate or equivalent (GED).
  • Medical office administrative responsibilities for 1+ year
  • Experience with Microsoft products to include Outlook, Word, Excel, Teams
  • Knowledge of Medicare, Medicaid and third-party reimbursement requirements.
  • Strong organizational skills.
  • Strong attention to detail.
  • Demonstrates professionalism, empathy, and commitment to patient-centered care.
  • Knowledge or ability to learn Hospice policies and procedures.
  • Ability to exercise independent judgement.
  • Acceptance and adaptability to different social, racial, cultural and religious modes.
  • Ability to respond with a sense of urgency.

Nice To Haves

  • Prefer 1+ years' experience in Hospice administrative services.
  • Proficiency with electronical medical records system (EMR/EHR), Wellsky a plus.

Responsibilities

  • DME Ordering/Pick-up
  • Follow up and confirm of RN requests & Orders
  • Management / review of DocuSign documents
  • Review and assign incoming faxes
  • Admissions, Discharges, & all LOCs
  • Visit Statistics (Missed Visits)
  • Pharmacy reports
  • DME Complaints
  • IDG Agenda
  • Submit CNA Assignments/frequency changes
  • Month end reconciliation
  • Answer phone calls from patients/families/referral partners
