Your Care is Our Purpose. The Patient Care Coordinator assists with the delivery of patient care needs in the home whether internally or externally. Internal needs are needs or coordination of care within PurposeCare resources. External needs are provided by outside partners including Direct Medical Equipment (DME) ordering, lab work, home visiting physicians or other specialists. This role will work closely with third party vendors to identify efficient and cost-effective ways to deliver external patient needs. Provides assistance with day-to-day operations as needed.

Responsibilities

  • Understand home health internal workflow and all parts involved in the delivery of patient care.
  • Help answer and resolve incoming office calls and address various patient needs.
  • Set up visiting physician, specialist needs and Lab work needs for patient from start to finish ensuring each patient has been taken care of.
  • Develop a follow-up process when ordering patient external needs ensuring all requests are completed.
  • Track and monitor Lab results are received and faxed to appropriate ordering physicians.
  • Order DME needs from start to finish, ensure all required documentation and MD signatures are obtained and delivered to DME vendor in a timely manner.
  • Prepare Medicare supplies and trunk supplies for clinicians.
  • Maintain and organize inventory, including taking care of incoming packages, breaking down boxes, and stocking supplies.
  • Enter insurance orders through Parachute (wound care and DME), ensure proper diagnosis, and document in chart that order was placed.
  • Schedule Medical Couriers to pick up supplies and communicate high priority orders.
  • Refer patients to external partners podiatry and incontinence products.
  • Provide feedback on vendor performance and resolve or obtain additional partnership to improve patient services.
  • Track and monitor patient re-hospitalization and discharges
  • Maintains good working relationships and open lines of communication with personnel from physician offices, area hospitals, and clinics, etc. to assure flow of information and referrals for service.
  • Supports and adheres to policies and procedures.
  • Performs other duties as assigned by the Director of Operations and Administrator.
  • Must be knowledgeable about and consistently follow confidentiality and privacy policies and procedures.
