The Patient Care Coordinator Supervisor is responsible for overseeing the daily operations and management of a portion of the patient care coordination team (6-8 team members). This role ensures that patient care activities are efficiently organized and executed, facilitating seamless communication between patients, families, and healthcare providers. The Supervisor plays a critical role in maintaining high standards of patient care and satisfaction, ensuring compliance with institutional policies and procedures, and supporting the team in delivering compassionate and comprehensive care. The position requires strong leadership skills to manage a team of coordinators and to foster a collaborative environment that promotes excellence in patient care. The Patient Care Coordinator Supervisor will stay current with trends and developments in patient care coordination and serve as a role model and mentor to the PCC team. They will develop and maintain effective working relationships with healthcare providers, external stakeholders, and other health-related resources, and actively participate in departmental and interdepartmental committees. Additionally, the Patient Care Coordinator Supervisor will manage reports for the Quality and Clinical Management Committee (QCMC), including data quality analysis, and assist the pharmacy manager in planning and implementing quality improvement initiatives and/or plans of correction when needed. They are expected to provide staffing coverage and support the team as necessary.
