Working under the guidelines of Hamilton Health Center’s patient-centered model of care, the Patient Benefit Services Coordinator serves as a liaison for uninsured and underinsured patients, providing eligibility screening, enrollment assistance, education, and outreach related to Medicaid, CHIP, and Marketplace insurance programs. This role supports access to care by assisting patients with insurance applications, financial assistance options, and referrals while participating in in-reach and outreach activities to improve engagement in evidence-based care. Hamilton Health Center (Hamilton), established in 1969, is a Federally Qualified Health Center (FQHC) located in Harrisburg, PA.
