CCP specializes in providing comprehensive cardiology care and currently has over 30 locations throughout the Philadelphia region. We are currently hiring for a full-time Patient Care Representative at our Ridley Park, PA location. This position requires traveling to our Glen Mills, PA, and Springfield, PA offices. Please note this role is entitled to a $500 sign-on bonus.

Requirements

  • High School Diploma or GED required
  • 1–2 years of experience in medical office or hospital setting preferred.
  • Familiarity with EMR systems (e.g., EPIC, GPMS, Centricity).
  • Strong organizational and multitasking abilities.
  • Excellent communication and interpersonal skills.
  • Ability to handle sensitive patient information with discretion and
  • confidentiality.
  • EMR navigation and chart review
  • Document scanning & faxing
  • Microsoft Office (Word, Outlook, SharePoint)
  • High level of accuracy and attention to detail
  • Understanding of medical terminology and referral processes

Responsibilities

  • System & Software Access:
  • Introduction and orientation to CCP computer systems and SharePoint.
  • Setup and manage Windows, email, and Outlook accounts.
  • Configure Outlook signatures and manage out-of-office replies (email &
  • EMR).
  • Obtain and maintain all necessary logins, passwords, and user credentials.
  • EMR & Chart Management:
  • Navigate and review patient charts, including:
  • Chart summary
  • Office notes
  • Test results
  • Orders
  • Phone notes
  • Manage flags, phone notes, and administrative messages.
  • Change referring physicians and care locations as needed.
  • Print orders and understand EMR workflow (Admin Hold, In-Process, etc.).
  • Scan and preload documents into patient charts.
  • Deactivate charts for deceased patients or those who have left the
  • practice.
  • Notify physicians appropriately about deceased patients.
  • Communication & Messaging:
  • Identify when to send a flag vs. phone note.
  • Route messages correctly to physicians or appropriate team members.
  • Hospital Coordination & Records Management:
  • Access and navigate hospital systems (Mainline, Penn, Jefferson).
  • Retrieve, request, and scan medical records for NP or TC/DC from hospital
  • scheduling.
  • Understand forms needed for each type of patient transition and complete
  • them accurately.
  • Ensure all preload documents are scanned to S drive no later than 2 days
  • before appointments.
  • Operational Support & Workflow Understanding:
  • Understand full TC (Transition of Care) workflow for both EMR and GPMS.
  • Explain diagnostic tests offered and appointment durations.
  • Understand frozen schedules and implications.
  • Be familiar with provider numbers, office locations, and referral
  • requirements.
  • Perform document faxing through Biscom.
  • Handle S-drive uploads and management for preload.
  • Physician Support:
  • Complete daily assigned physician tasks.
  • Participate in weekly fax day responsibilities.
  • Appropriately file and manage incoming documents.
  • Scheduling office visits requested by physicians

  • If you are hired, and assuming your employment is in good standing, you will receive the sign-on bonus. The bonus will be paid to you no later than 30 days following your start date. In the event that you voluntarily terminate your employment or the Company terminates your employment for cause within 6 months of your date of hire, you will be required to repay the full amount of your sign-on bonus.
