About The Position

To work as a proactive member of the AccessPoint Team, providing meaningful individualized treatment services and supports that encourage children with special health care needs and their families to experience personal growth and reach their fullest potential.

Requirements

  • Is at least 19 years of age.
  • High School Diploma or GED required and two years experience working with children with special health care needs or a combination of experience, education, and training working with children with special health care needs and their families or an Associates Degree in a related field.
  • Willingness and ability to complete and perform CPR and Non-Violent Physical Crisis Intervention (NCPI).
  • Willingness and ability to complete PASS Training Curriculum.
  • Valid Drivers License and acceptable driving record required.
  • Access to and provision of an insured automobile on an ongoing basis.
  • Bilingual

Responsibilities

  • To provide supports and services consistent with those outlined in the PASS Plan and Individual profile.
  • To ensure that all children with special health care needs and families supported are treated with Dignity and Respect.
  • To support children with special health care needs and their families in developing a positive self image.
  • To treat each individual within the family as persons who have unique needs and wants.
  • To support children with special health care needs and their families to function to they're fullest potential.
  • To support children with special health care needs and their families to participate in their home and community in ways that are meaningful to them.
  • To encourage children with special health care needs and their families to maintain positive relationships.
  • To work with PASS support team in developing consistent and integrated support techniques, methods and programs.
  • To attend and actively participate in team meetings.
  • To support and carry out all team decisions consistently and appropriately.
  • To attend in-service trainings as determined by the Program Director.
  • To gather and submit data related to achieving PASS goals in a timely manner.
  • Home-Based Workers are responsible to maintain the following complete and legible contemporaneous records.
  • To complete preparation materials and participate in the semi-annual planning meetings, if requested (PASS status update and renewal, IEP.).
  • To ensure the optimal health and safety of children with special health care needs receiving supports.
  • To adhere to all safety policies, including safe driving and seatbelt policies.
  • All other duties as assigned and/or as identified in the PASS Treatment Plan and Individual Profile.
