Requirements

• Must hold (or be eligible to obtain) a Physical Therapist license in the state of Illinois
• One year experience as a licensed Physical Therapist, required

Responsibilities

• New patient evaluation and follow up visits with ongoing POC management
• Complete all documentation in a timely manner
• Actively participate in In-House Continuing Education presentations
• Participate in marketing activities, as needed
• Maintain compliance with all company policies and procedures
