LightWorks Outpatient Counseling and Wellness by Shineforth part-time outpatient therapist will provide in-person and telehealth, compassionate, evidence-based, therapy services to individuals ages 5-26, including guardians/families, who are demonstrating mental health needs. The outpatient therapist will collaborate closely with the Associate Director of Outpatient Services and with a multidisciplinary clinical and administrative team to ensure seamless, integrated care. A minimum availability of three days per week and an average of 12-15 billable hours/week is required. Key responsibilities include clinical intake and assessment, individualized treatment planning, individual, group, and/or family therapy. Collaboration with multidisciplinary treatment team. Coordination with external health care providers/community resource agencies, as needed, on treatment strategies and comprehensive discharge planning. Clinical Care Conduct initial assessments and evaluations to identify and diagnose mental health conditions Develop individualized treatment plans based on client needs and goals, regularly assess client progress and updating as needed Conduct individual therapy, including group and family therapy sessions, as clinically indicated Communication with legal guardians/caregivers between sessions as clinically indicated Commitment to utilizing current, evidence-based treatment modalities Participate in group supervision, training/professional development opportunities and required administrative meetings Work closely with the Psychiatric Mental Health Nurse Practitioner to enhance conceptualization of client, need for ancillary services, and improve overall treatment options. Documentation & Administrative Duties Complete all clinical progress notes and other required clinical documentation (intake documentation, intake assessments, monthly reports as applicable, etc) within established deadlines in the electronic health record Ensure clinical documentation is clinically relevant, complete and in alignment with agency, licensing, accreditation, and insurance policies and procedures Provide discharge summary including plans for continued care at end of service Maintain meticulous and confidential medical records in compliance with HIPAA Participate in ongoing quality improvement initiatives Collaboration & Care Coordination Work closely with Psychiatric Mental Health Nurse Practitioner, Associate Director of Clinical Services, administrative staff, and other multidisciplinary team members Ensure appropriate attention to crisis management concerns by promptly responding to emergencies in alignment with program policies and procedures, utilizing verbal de-escalation techniques and other agency behavior management practices Provide client and family education related to diagnosis, treatment, and integrated behavioral health Refer to internal and external programs and providers as clinically appropriate RESPONSIBILITIES OF ALL EMPLOYEES-PROFESSIONAL COMPETENCIES Customer focus – Demonstrates that internal and external customers are a high priority.
