About The Position
We are seeking a Nurse Practitioner to join our growing palliative care program. This is an opportunity to make a meaningful impact by supporting patients with complex, chronic, or serious illnesses in a collaborative, patient-centered environment. Position Details: Part-Time: 20 hours/week (non-benefited) Full-Time: 32–40 hours/week (benefited)
Requirements
- Experience preferred but willing to train new Nurse Practitioners!
- Active Nurse Practitioner license in Wisconsin
- Strong clinical assessment, documentation, and care planning skills preferred
- Experience with eClinicalWorks (eCW) EMR preferred (willing to train)
- Excellent communication and teamwork abilities
Responsibilities
- Conduct comprehensive palliative care visits, including assessment, diagnosis, and management of symptoms
- Complete F2F encounters for hospice patients as needed
- Develop and implement individualized care plans in collaboration with the interdisciplinary team
- Prescribe medications and coordinate care for patients with advanced or chronic illnesses
- Accurately document all encounters in the EMR
- Communicate effectively with patients, families, and other healthcare professionals
