Functioning primarily as part of the palliative care team, the Palliative Care Social Worker is responsible to support children and families experiencing living loss, anticipatory grief and grief due to bereavement in the hospital setting. Helps children, adolescents, and their families understand their treatment plan and be vocal about their needs; manage the stresses of debilitating physical illnesses, including emotional, physical, mental, social and spiritual; overcome crisis situations; connect to other support services; and navigate planning for end-of-life care. Acts as a bridge between palliative and bereavement so that families experience exceptional continuity of care.
