Palliative Care RN Clinical Coordinator

The Palliative and Hospice Care Clinical Coordinator is an office-based role, collaborating with inpatient clinical teams to ensure that patients receive the right care, at the right time, and according to the patients’ goals and wishes. Specifically, this role advocates for appropriate referrals to palliative and hospice services, and also facilitates educational offerings to increase clinician awareness of palliative and hospice care options.
Requirements
- Experience in hospice and palliative care required
- Current Maine RN Certification
- Certification in Hospice and Palliative Care required within 2 years of hire.
- Ability to communicate clearly in English, speak effectively by telephone and written word, operates a computer efficiently, and uses translation services appropriately.
