About The Position

JR180366 Palliative Care Case Manager CareBridge Health is a proud member of the Elevance Health family of companies, within our Carelon business. CareBridge Health exists to enable individuals in home and community-based settings to maximize their health, independence, and quality of life through home-care and community based services. The Palliative Care Case Manager is responsible for coordinating referrals, education, and interdisciplinary support for hospice and palliative care services to ensure that members with serious or terminal illnesses receive appropriate care. The role supports accurate member understanding of care goals and facilitates engagement with relevant community resources. Location: Virtual: This role enables associates to work virtually full-time, with the exception of required in-person training sessions (when indicated), providing maximum flexibility and autonomy. This approach promotes productivity, supports work-life integration, and ensures essential face-to-face onboarding and skill development. How you will make an impact: Identifies eligible members for hospice and palliative care and ensures their successful admission to services. Conducts serious illness conversations with members and caregivers to clarify care goals and document preferences. Educates members, caregivers, and clinical staff on hospice and palliative care options and benefits. Coordinates ongoing care and communication between CareBridge and community palliative organizations. Tracks referrals and develops strategies for process improvements in member engagement and service delivery. Maintains supportive relationships with both internal and external stakeholders to ensure continuity of care. Develops and implements educational materials and training programs for internal teams. Assists with process design and improvement efforts unique to the palliative care navigator function.

Requirements

  • Requires a bachelor’s degree in a related field, such as BSN, and a minimum of 3 years of clinical experience in a hospice or palliative care organization; or any combination of education and experience, which would provide an equivalent background.

Nice To Haves

  • RN certification, APC, or IFOC certification preferred.
  • MSW, LCSW, or MDiv preferred.
