About The Position

Trinity Health Of New England At Home serves patients and their loved ones by providing a variety of in-home services including but not limited to nursing, social work and physical/occupational/speech therapies. Position Purpose: The Palliative Care RN is responsible for patients and families in the palliative care program. Provides Palliative care clinical consultation. Position Details: Schedule: Monday - Friday, 8 am - 5 pm, one weekend every 4 weeks and two rotating holidays per year. NO ON CALL REQUIREMENTS! Coverage Region: Enfield Bonus: $10,000 sign-on bonus paid out over two years. This position is 100% home care providing services in in patients private homes. What you will do: Collaborates with manager in the development of performance measurements for the Palliative Care Program to ensure continuous quality improvement; ensures implementation Motivates/supports employees for exemplary performance on internal and external customer service, productivity, and accuracy Provides statistical/budgetary data to appropriate Palliative care program. Works collaboratively with all departments to ensure execution of quality delivery of services Ensures that stakeholders are satisfied with services provided; improvement plan developed when necessary. Develops tracking outcomes of enrolled patients and ensures information is available for review A commitment to quality palliative care provision and transition to hospice when appropriate Exercises sound clinical judgment in oversight of Palliative Care Program Knows, understands, incorporates, and demonstrates the Trinity Health Mission, Vision, and Values in behaviors, practices, and decisions. Requirements

• Graduate of an approved nursing education program.
• Current Registered Nurse licensure in the state of practice.
• CPR certification
• Home care experience, strong clinical knowledge base, Hospice experience
• Computer skills, strong interpersonal and organizational skills.
• Palliative Care experience, demonstrate ability to use problem solving, critical thinking and priority setting skills.
• Must be able to work independently and on a team
• Ability to perform diversified duties with time limitations
• Ability to develop, coordinate and evaluate programs in Palliative care and end-of-life care.
• Valid driver’s license and reliable transportation required

Nice To Haves

• Hospice/Palliative Care certification preferred.
