About The Position

The Nuvance Health Medical Practice Division of Pain Management has an excellent opportunity for a Pain Management Nurse Practitioner to join our comprehensive Neurosciences Department in Carmel, NY. Putnam Hospital Center (PHC), in Carmel, New York, is a 165-bed community hospital. With a staff of more than 300 physicians, PHC offers the highest quality medical, surgical, psychiatric, oncology, pediatric, orthopedic, obstetrical/gynecological, and intensive care, as well as 24-hour emergency services and home health care. Putnam Hospital Center is in a beautiful suburban location, an hour north of New York City. Requirements

  • Master’s degree in Nursing with a focus on Nurse Practitioner studies.
  • Current Nurse Practitioner licensure and certification in a relevant specialty (e.g., Pain Management, Family, or Adult).

Nice To Haves

  • Previous experience in pain management, palliative care, or a related field preferred
  • New graduates will be considered

Responsibilities

  • Global management of patients with pain conditions
  • Conduct thorough evaluations of patients with pain conditions, including detailed medical histories and physical examinations.
  • Develop and implement individualized treatment plans based on patient assessments, incorporating pharmacologic and non-pharmacologic therapies.
  • Utilize various modalities for pain relief, including medication management, physical therapy referrals, interventional procedures, and complementary therapies.
  • Educate patients and families about pain management options, coping strategies, and lifestyle modifications to improve quality of life.
  • Schedule and conduct follow-up appointments to monitor treatment efficacy and adjust plans as needed.
  • Maintain accurate and timely documentation of patient assessments, treatment plans, and outcomes in accordance with healthcare regulations.
