About The Position

Reporting to the Program Coordinator, this position is responsible for providing case management services to participants aged 15-18 years old. This position is responsible for recruiting, conducting assessments, enrolling, and securing resources for participants. This position is responsible for managing the process of service delivery from enrollment to goal achievement to participants ensuring that quality outcomes are achieved through advocacy and well-established rapport with persons served.

Requirements

  • Bachelor’s degree level. Preferably in Human Services or Criminal Justice and two (2) years of direct service experience.
  • The ability to communicate with individuals from diverse backgrounds and the ability to adapt to working effectively in high stress and fast paced environments.
  • Proficient in Microsoft Office 365 and excellent verbal and written communication skills.
  • Incumbent must have the ability to work with individuals with a criminal history.
  • Ability to function as part of the team and willingness to be responsive to the needs of the target population.

Nice To Haves

  • Demonstrated knowledge of the criminal justice system, human behavior, and social work skills are preferred.

Responsibilities

  • Assist participants to develop and coordinate Individual Service Plan which entails: Employment, Education, Substance, and Mental Health.
  • Documents services and ensures that records are completed according to established policies and procedures.
  • Implements initial informational gathering and assists participants in assessing and evaluating goals and or training goals for their Individual Program and Career Plans.
  • Coach and encourage youth participation through employment, education, and career progression.
  • Counsels’ participant on personal, financial, employment behavioral, crisis, substance abuse, educational and training matters.
  • Conducts on site visits to prisons, detention centers, participant’s home and work locations for monitoring and support to record their progress.
  • Help participants build positive social networks through service-learning projects.
  • Work closely with corrections officer and courts to advocate and provide support to participant within due process.
  • Must have general knowledge of the criminal justice system.
  • Be able to assist participants with completing documents as it relates to judiciary procedures such as expungements and diversion program.
  • Participate in program related trainings and other training programs as required.
  • Keep supervisory staff apprised of gaps in services available and assist in services needed for client’s success.
  • Prepare required reports, surveys and documentation as required by funders and program.
  • Cultivate and sustain professional relationships with partnering agencies.
  • Maintain caseload with accuracy and professionalism.
  • Be able to assess and document needs through one-on-one meetings with participants.
  • Must have the ability to handle multiple priorities simultaneously.
  • Must be able to establish and develop partnerships with local community agencies.
  • Perform other duties as assigned.
