About The Position

The Outreach Therapist position offers a perfect mix of autonomy and team solidarity, supported by daily team meetings and weekly supervision focused on collaboration and professional growth. The role provides flexibility to create a day-to-day appointment schedule. Therapists connect with clients in the community at their homes or other community settings, ensuring that "Every Day is a New Experience!" This approach fosters long-term relationships with clients that have a lasting impact. The Assertive Community Treatment Team (ACTT) serves adults 18 years of age and older, providing intensive community-based treatment, support, and rehabilitation to individuals with a serious persistent mental illness (SPMI) or a dual diagnosis of SPMI and a Substance Use Disorder, who are determined to require a high level of service needs.

Requirements

Master’s of Social Work.
2+ years working with identified populations.
LMSW required.
Valid Driver’s License.

Nice To Haves

Experience working with families, providing family therapy; individual and group.
LCSW preferred.

Responsibilities

Conducts Clinical and Risk Assessments, including: LOCUS, Co-occurring Screens, DLA-20.
Collaborates with individuals to create individualized, person centered treatment plans and documents clients progress during treatment.
Provides individual and group therapy.
Use Motivational Interventions to assist clients in meeting their goals and moving along their paths to recovery, utilizing stage wise interventions.
Provide psycho-educational rehabilitation services group and individual for recovery from psychiatric and substance abuse disorders.
Support individual with linking and engaging with natural (family, friends) or community supports.
Develop crisis plans for clients and work with other staff to respond to crisis situations as needed.
Provide skills teaching, such as assisting clients in obtaining/ maintaining housing and utilities; learning how to use transportation services, planning and preparing nutritional meals, engage and communicate with family and friends, be safe in the community, care for children and other family members, and solve everyday problems that arise.
Provide intensive skill building interventions and support for self-management, self-advocacy, health and well-being, relapse prevention, and using community resources.
Participate in family education and support activities.
Maintain documentation in compliance with agency and program standards.
Participates in staff meetings, supervision, trainings and other meetings as assigned.
Additional duties as required.
