Will assist in implementing best practice interventions focused on improving the well-being, reducing stress levels, and resiliency of the targeted populations to be served. These interventions will use academic settings, health fairs, community, and peer interactions to target pregnant, post-partum women, or male or females with a child under the age of 6 with open CPS case. Outreach Navigator will also facilitate evidence-based curriculum for parenting sessions for our targeted population.

Requirements

  • Ability to express self, in English, in a clear, concise, understandable manner, both orally and in writing.
  • Bilingual (English/Spanish).
  • Reflect a pleasant attitude that represents Aliviane, Inc in a positive manner within the community at large, and maintain a professional attitude to the employees, clients and public at all times.

Responsibilities

  • Conduct extensive, culturally sensitive outreach for pregnant, post-partum women or men and women with a child under the age of 6 with open CPS case.
  • Outreach Navigator will determine the type of intervention provided such as physical, or educational support.
  • Outreach Navigators will screen individuals for mental health, substance use, and Social Determinants of Health.
  • Referrals will be given to the participants to ensure they receive the support and services they need.
  • Outreach Navigators will distribute educational pamphlets, posters, lectures, and conversations on mindfulness, tips to reduce stress and improve well-being.
  • Participate in community outreach events, and health fairs to screen and refer individuals for services as necessary.
  • Outreach navigators along with community partners will work together to promote resiliency, social and community connectedness, and healing for individuals at risk for substance misuse.
  • Outreach Navigator will also facilitate evidence-based curriculum for parenting sessions for our targeted population.
  • Outreach navigators will travel to rural areas.
  • Have knowledge of the agency's programs in order to refer individuals who seek information.
  • Maintain working knowledge of personnel and office policy and procedures.
