About The Position

The Outreach Services Associate is responsible for contacting and scheduling patient/client appointments for consultations, meetings with community agencies, and/or diagnostic tests. This role also involves setting up and maintaining files and records, and collecting and organizing statistical reports.

Requirements

• High School Diploma or equivalent required.
• 0-1 years of relevant experience, required.

Responsibilities

• Contacts patients; schedules appointments for consultations, agency meetings and/or diagnostic tests.
• Collects and organizes data required for statistical reports and completes reports, as required.
• Attends and participates in meetings as appropriate.
• Sets-up files and maintains records.
• Provides incoming and in-process orientation.
• Based on position, may be required to provide bi-lingual translation and/or guidance.
• Performs related duties as required.
