About The Position

PRIMARY PURPOSE: The Outreach Program Coordinator supports the Director of Outreach and Community Engagement in coordinating outreach services and community engagement across all FoundCare programs. This position assists with the linkage workflow across programs, manages outreach schedules, and tracks deliverables across all grants. A Certified Application Counselor (CAC) certification is required to facilitate insurance enrollment processes.

Requirements

  • Knowledge of outreach methodologies and familiarity with community locations frequented by the target population.
  • Ability to comprehend and apply information from training programs, conferences, and professional literature.
  • Current knowledge of health issues, including HIV/STD/TB transmission and prevention strategies.
  • Ability to communicate effectively using the English language, with or without the use of auxiliary aids or services.
  • Effective interpersonal skills to maintain rapport with clients, staff, and members of the community.
  • Outstanding customer service skills and the ability to interact and work with diverse populations.
  • Excellent organizational skills.
  • Proficiency in Microsoft Office Suite, including Outlook, Word, and Excel, and ability to navigate electronic health record systems.
  • Awareness that FoundCare provides comprehensive health education, which may involve exposure to explicit content as part of the agency’s mission.
  • Ability to endure short, intermittent, and/or long periods of sitting and/or standing in performance of job duties.
  • Use standard office equipment and technology in the course of daily duties.
  • Ability to lift and carry objects weighing 25 pounds or less over distances of 100 yards and travel to various community locations for outreach activities.
  • Ability to move, stack, and store HIV and substance abuse prevention materials and supplies in storage area, including lifting materials and placing on shelves at levels 5’ 6” and lower.
  • Ability to travel to various community locations throughout Palm Beach County for outreach.
  • Ability to travel to other FoundCare locations and perform job duties.
  • Ability to travel to other locations to attend meetings, workshops, and seminars, plus travel to other FoundCare departments and FoundCare conference rooms.
  • Possession of a bachelor’s degree from an accredited university in social work, public health, behavioral sciences, or education services, with one (1) year of related experience in care coordination.
  • Willingness to work a flexible schedule, including evenings and/or weekends.
  • Valid Florida driver’s license, automobile insurance, and reliable transportation.
  • Certified Application Counselor (CAC) certification required for facilitating insurance enrollment.
  • This role is not eligible for visa sponsorship.

Nice To Haves

  • Possession of a master’s degree from an accredited university in public health.
  • Experience delivering services in a medical or social services setting.

Responsibilities

  • Collaborate with the Director of Outreach & Engagement to collect and provide all required information to complete monthly, quarterly, and annual reports.
  • Collect required data for program evaluation, generate weekly and monthly activity reports, and enter and review testing data in Evaluation Web.
  • Ensure proper documentation of services provided under the program.
  • Assist the Director of Outreach & Engagement in training new staff members, including providing orientation to field activities.
  • Assist the Director of Outreach & Engagement to implement program policies and procedures.
  • Assist with the linkage workflow, ensuring smooth transitions and access to necessary services.
  • Participate in grant activities throughout targeted areas, ensuring the program team meets grant deliverables.
  • Track deliverables across all grants supporting outreach and monitor reporting requirements.
  • Provide counseling and testing services for HIV and other relevant health issues as needed.
  • Coordinate and ensure the client’s linkage to medical and supportive services across all programs.
  • Coordinate referrals to PrEP and PEP programs for individuals who test negative for HIV.
  • Coordinate weekly schedules for outreach activities to ensure effective service delivery.
  • Ensure documentation of all care provided, directly or indirectly, including referrals, services, and consultations.
  • Ensure individuals who test positive or are re-engaged in care are referred to a medical provider within 72 hours for the Test and Treat program.
  • Maintain current knowledge of HIV treatment and risk reduction strategies and resources available to assist individuals.
  • Assure and maintain client and family confidentiality.
  • Establish positive working relationships with FoundCare staff, gatekeepers, community partners, local authorities, and staff from other agencies to facilitate access to program services for the targeted community.
  • Maintain relationships with community partners.
  • Assist with managing the FoundCare general events calendar.
  • Photographically document Outreach activities for promotion on FoundCare channels.
  • Assist with generation of Outreach activity flyers.
  • Comply with all organization policies and procedures, including street and community outreach.
  • Perform other duties as assigned.
