About The Position

The Patient Navigator will play a critical role in guiding patients through their healthcare journey. The primary responsibilities will include assisting patients with check-in and check-out processes, scheduling and managing appointments, and providing support with insurance eligibility and applications. The Patient Navigator will serve as a key point of contact for patients, ensuring they understand and complete necessary paperwork and applications. The goal is to facilitate a smooth and supportive experience for patients from their initial visit to their follow-up care. The Patient Navigator is also expected to actively participate in and support quality improvement activities and the clinical practice team efforts emphasize the importance of the individual patient and putting the patient’s needs first.

Requirements

  • High School Diploma or equivalent – required
  • Spanish fluency – required
  • Faith to believe that Hope is a ministry of God, exemplifying the love of Christ, and held and directed by the Holy Spirit.
  • Demonstrated passion for community health care.
  • Excellent communication skills, both written and verbal.
  • Proven ability to maintain integrity in a fast paced and high-demand environment.
  • Proven ability to manage performance goals.
  • Proven teambuilding and organizational skills.
  • Ability to interact with people from all ethnic backgrounds, ages, and lifestyles.

Nice To Haves

  • Community Health Worker (CHW) Certification – preferred
  • Previous experience in a healthcare or administrative role - preferred
  • Familiarity with electronic medical records (EMR) systems and insurance portals – preferred

Responsibilities

  • Guide patients through various processes, including check-in, intake, and check-out procedures.
  • Schedule and coordinate follow-up appointments, ensuring timely and accurate bookings.
  • Remind patients of upcoming appointments via phone calls.
  • Handle phone calls to and from patients, addressing inquiries and providing information as needed.
  • Communicate referral information to patients, ensuring they understand the next steps in their care.
  • Assist patients with understanding their insurance benefits and coverage.
  • Verify patient identification to ensure accurate and secure records.
  • Assist patients with completing intake paperwork and applications, including the sliding fee scale program application.
  • Educate patients on the importance and completion of required documents.
  • Verify patient insurance eligibility and navigate insurance portals to obtain necessary information.
  • Maintains and ensures patient confidentiality at all times
  • Maintains a courteous and professional demeanor at all times; greets individuals and patients as they enter and exit the clinic; facilitates appropriate communication and interpersonal skills with all staff members, volunteers, and patients
  • Maintains safe, secure, and healthy work environment by establishing and following standards and procedures; complies with legal regulations
  • Ensures front office and clinic policies and procedures (as applicable) are followed at all times.
