The Outreach Navigator’s primary role is to enhance the likelihood of success, identify and reduce barriers for SMI clients and/or eligible MHBG individuals and connect them with needed services and resources, including COVID-19 Personal Protective Equipment (PPE) supplies and education. The Outreach Navigator will work with and outreach SMI individuals in need of additional support and engagement case management, and ongoing wrap around supportive services. Services include COVID-19 PPE supplies, and COVID-19 education and resources. The Outreach Navigator will provide services to individuals pre/post incarceration, as well as individuals with a recent Crisis Mobile Team (CMT) activation. The Outreach Navigator maps out the most appropriate next steps for individuals in need of assistance with service access. The Outreach Navigator is part of a multi-disciplinary team made up of internal and external stakeholders. The Outreach Navigator will document, and track required patient interactions according to the Arizona Administrative Code (AAC), Arizona Department of Health Services/Department of Behavioral Health Services (ADHS/DBHS) Provider Manual, Commission on Accreditation of Rehabilitation Facilities (CARF), Community Bridges Policies and Procedures, and specific for the program.
